Insurance for CHIROPRACTIC CARE

Dr. Leighton is in network with Blue Cross Blue Shield PPO (not BlueChoice or Medicaid) plans and UnitedHealthcare PPO plans for chiropractic care only. Dr. Leighton is an out-of-network provider for all other health plans for chiropractic care.

Since there are numerous plan options and coverage varies widely, please call your customer service representative (contact information on the back of your insurance card) to find out if your plan covers care with me.  Your insurance company will ultimately determine treatment coverage and potential reimbursement. Many insurance plans offer out-of-network coverage. If this is the case, a super-bill can be created for you to submit to your insurance plan for direct reimbursement.

If your insurance plan is billed for your visit, the following codes will be billed based on your scheduled appointment type:

  • New Chiropractic Visit (all types): 99203, 98940/98941
  • New Acupuncture Visit: 99202, 97810, 97811
  • Return Chiropractic for Pediatric Oral Tie Management: 98941, 98943, 97110 (99212 re-exam)
  • Return Chiropractic for Pregnancy/Postpartum Care: 98940, 97110 (99212 re-exam)
  • Return Acupuncture Visit: 97810, 97811 (99212 re-exam)

Due to contractual obligations with your insurance carrier, any co-insurance, copay, or deductible amounts must be collected from you after your claim has been finalized.
